Make sure that the following credentials have been submitted to the admissions office before proceeding to the next step:

  1. High School Report Card / Form 138
  2. Certificate of Good Moral Character
  3. Pictures: 1x1 and 2x2 colored (2 pcs. each)
  4. Long brown envelopes (2 pcs.)

Important:
In cases where credentials are not complete or cannot be submitted yet, secure a Promisory Note from any Admissions Staff and fill-out the form.

 

Entrance Examination:

  1. Secure and fill-out an application form with your 1x1 picture (please write your name at the back of all your pictures). Do not forget to sign in the Admissions Logsheet (please ask from any Admissions staff).
  2. Proceed to the Accounting Office for payment of Testing Fee (P100.00). Do not forget your Official Receipt and Application Form.
  3. After payment, proceed to the Guidance Office for testing. Your examination results will be given to you a few minutes after you take the examination. Do not forget to ask for your entrance exam result before proceeding to the Admissions Office for enlistment.

*For students who have taken On-site Entrance Examinations in their respective schools, please present your letter of acceptance sent by AMA.

 

Enlistment:

  1. The attending admissions staff will assign a student number specifically created for you. Do not forget your student number as this will be your identification everytime you transact with the school until you graduate.

  2. Secure a plotting form, course curriculum and a recommendation note from the Admissions Director for subject crediting to the Registrar or Deans, interview and advising to respective College Deans.

  3. Proceed to your respective colleges for interview, subject crediting and advising of deans.

    College of Computer Studies: Engr. Emelin Magada
    College of Engineering: Engr. Samuel Arquero
    College of Business Administration and Accountancy: Dr. Victor Mariano / Ms. Exortacion Alcaraz
    College of Arts and Sciences: Dr. Arsenia Casauay

  4. Deans of respective colleges will evaluate your credentials (TOR/TCG/Report Card) and will conduct an interview. If applicant is qualified for admission, the dean will advise you of subjects recommended for enlistment for this semester.

    Block sections will be assigned to incoming freshmen by the Deans / Area Coordinators.


  5. Enlistment for new freshmen students under the block section will be done in their respective colleges.

 

Important:
In case subjects recommended for enlistment are in conflict or are already closed, please refer to other subject offerings. The schedule of subject offerings are posted in the bulletin board of your colleges and the pathwalk fronting the CB Building. Approach any Admissions staff for assistance regarding enlistment of subjects and schedules.

 

Assessment, Payment and Printing of C.O.R.

  1. After enlistment, you are now ready to enroll. Please proceed to the Admissions Office and secure a Scholarship Cash Credit from the Admissions Director. This is an additional incentive we give to our new students to curtail enrollment expenses.

  2. Proceed to the Accounting Office for assessment and payment of fees.

    Assessment:
    The assessment form is a presentation of all your subjects this semester and the corresponding fees. It indicates breakdown of fees and the schedule and breakdown of payments.

    For full payment:
    Cash payments automatically merit 5% discount from your tuition fees.

    For partial payment:
    1. Secure a promisory note from any Admissions staff and fill out info needed;
    2. Proceed to the Accounting Office for fee computation and breakdown of fees;
    3. For new students, the Admissions Director will sign in behalf of the OSA Director's recommending approval
    4. Proceed to the Office of the Vice President / School Director for approval.

    Payment:

    1. Proceed to the Accounting Office (Cashier) for payment of Fees.
    2. After payment Cashier will issue an Official Receipt representing payments made for your enrollment.
    3. Request for your Certificate of Registration (COR) or what our students fondly call the Blue Form. Do not lose the COR as this is proof of your enrollment.
    4. Secure an application form for ID from the Admissions Office and proceed to the Office of Student Affairs for submission.
